HRS is here to support those with rheumatic related diseases for those in the Hawaii community. Members of HRS can apply for a maximum of $100 per calendar year for one to two individuals. Payment should be made to the Rheumatologist provider who recommended the patient to apply for the HRS fund.

Qualifications for Membership:

Licensed Rheumatologists or other care provider:  Those who are current and active members of HRS can recommend patients to apply for the fund.

               Current and active members are defined as: One who has                 made a donation of at least $100 for the current calendar                  year and has attended at least one HRS event for that year. 

Any Qualified Individual: A person who has been diagnosed with a rheumatologic disorder and is in need of financial assistance.

MEDICAL FINANCIAL SUPPORT FUND

The purpose of the HRS Medical Financial Support Fund is to provide for patients with rheumatologic diseases. HRS understands that those with rheumatologic diseases often face financial difficulties due to the medical bills required for treatment. With the ever increasing costs of medical care, the HRS Medical Financial Support Fund seeks to aid in costs for the uninsured, or those in need to fund co-pays and other medical bills one has received. The HRS Medical Financial Support fund can grant a maximum of $100 (one-time grant) to qualified individuals.


Qualified individuals

  1. Licensed Rheumatologists or other care provider who are current and active members of HRS can recommend patients to apply for the fund.

                Current and active members are defined as: One who has made a donation of at least $100 for    

   the current calendar  year and has attended at least one HRS event for that year.


      2. Any Qualified individual: A person who has been diagnosed with a rheumatologic disorder and is  

          in financial need. The need for financial assistance can be corroborated through proof of income

          from federal tax returns of the previous year.



All patients are required to complete an application form with other appropriate documents demonstrating financial need, and fax to 808-523-2912.

Review and approval of application can take up to 2 weeks. If your application is not approved, you will be notified for the reason of denial. Your Rheumatologist’s office will be notified of your application. Members of HRS can apply for a maximum of $100 per calendar year for one to two individuals, with the total amount for two patients not exceeding $100 (ex. one individual can receive $55 and the other $45). Payment should be made to the Rheumatologist provider who recommended the patient to apply for the HRS fund. If the applicant has already paid out of pocket for qualified medical expenses, they will be reimbursed directly.

1. Application Form (Attached below)

2. Applicant’s current balance statement with the Rheumatologist office OR a high balance EOBs (Explanation of Benefits), which includes the primary insurance payment and adjustment.
